Abstract: Мета роботи – оцінити показники цитокінової регуляції при туберкульозі легень (ТБЛ) з різною чутливістю до протитуберкульозних препаратів залежно від функціонального стану щитоподібної залози (ЩЗ) та рівня кортизолу. Матеріали та методи. Вивчали показники активності окремих інтерлейкінів (ІЛ), зокрема прозапальних ІЛ-6 та ІЛ-18 і протизапального ІЛ-10, у сироватці крові хворих на ТБЛ за допомогою імуноферментного аналізу. Виокремлено дві клінічно-патогенетичні групи залежно від чутливості до протитуберкульозних препаратів та функціонального стану ЩЗ. Результати та обговорення. Результати дослідження засвідчили, що у хворих зі збереженою секрецією ЩЗ, незалежно від варіанта чутливості, визначається вірогідне зростання рівня ІЛ-6, ІЛ-18 та ІЛ-10. Установлено, що в пацієнтів із тиреоїдною дисфункцією та гіперкортизолемією рівень ІЛ-6, ІЛ-18 та ІЛ-10 вірогідно нижчий (р < 0,05), ніж у групах зі збереженою тиреоїдною активністю. Висновки. У пацієнтів із ТБЛ зі збереженою чутливістю до протитуберкульозних препаратів та в резистентних хворих з нормальною секрецією ЩЗ визначається вірогідне зростання рівня як прозапальних цитокінів ІЛ-6, ІЛ-18, так і протизапального ІЛ-10, коефіцієнт балансу зрушений у бік прозапальної активності. При ТБЛ із тиреоїдною дисфункцією та гіперкортизолемією визначається зростання рівня як ІЛ-6, ІЛ-18, так і ІЛ-10, однак їх рівень вірогідно нижчий, ніж у групах зі збереженою тиреоїдною активністю. Objective. To evaluate the indices of the cytokine regulation of pulmonary tuberculosis with varying sensitivity to anti-TB drugs according to thyroid function and Cortisol levels. Materials and methods. The levels of interleukins (IL) IL-6, IL-10, IL-18 in serum of patients with pulmonary tuberculosis (TBP) has been studied with immunoenzymatic analysis. Two clinical and pathogenetic groups has been defined depending on the sensitivity to anti-TB drugs and thyroid gland (TG) function. Results and discussion. The obtained results showed that in patients with preserved thyroid secretion, the possible increase of the IL-6, IL-18 and IL-10 levels has been defined regardless of the sensitivity variant. In patients with thyroid dysfunction and high level of Cortisol, the levels of IL-6, IL-18 and IL-10 were significantly lower (p < 0.05) than in the group with preserved thyroid activity. Conclusions. Pulmonary tuberculosis with preserved sensitivity to anti-TB drugs and resistant patients with normal thyroid secretion is defined as the likely increase in the proinflammatory cytokines IL-6, IL-18 and antiinflammatory IL-10 ratio balance shifted toward the proinflammatory activity. Pulmonary tuberculosis with thyroid dysfunction and high level of Cortisol defined as increase in IL-6, IL-18 and IL-10, however, their levels arc significantly lower than in the group with preserved thyroid activity.
